· Registration fee is $100.00. Non Cobb County resident’s annual fee is $25.00.

· A new player fee includes an annual fee of $25.00 for Georgia State Soccer.

· First half fees due for individual players by August 1st, 2006 .

· NO refunds.

· Registration forms can be found online.

· All teams need to submit:

- Georgia State Adult Team Form (please list an alternate contact)

- Georgia State Individual Player Forms for each player

- Fees for each player

- Photos (1" by 1") for new players
